“Living Well: Chronic Disease Self-Management,” is a
six-week program emphasizing strategies to improve health – one step at a time. It is offered for people who have
arthritis, diabetes, heart disease, asthma, emphysema, obesity, depression, or other long-term health conditions.
Family member, friend or caregiver can also participate.
Participants will learn better ways of coping and managing their health by setting goals that are doable, finding support and solutions to problems, learning to manage stress and work in partnership with their health-care team.
